Student please apply by using your 10-digit application number (xxxxx-xxxxx) or
student ID number.

If you want to change any information after filling-up this form, please contact
the person in-charge from your faculty.

The full time student must bring the following documents to the Registrar's Office.
|
|
|
|
|
- Copy of Identification card or Passport
- Copy of Certificate or Diploma
- One photo of student with size 1 inch X 1 inch
|

|
|
Student must provide true, accurate and complete information in this form. Any false
information misrepresentation will have serious consequences.
